- - - - Insurance Forms
        
        Insurance forms are used by the clinics to determine treatment options and are normally color coded depending on the coverage type. The primary audience is the clinic staff so that they have proof that patient has coverage. Unintended audiences may include family members or friends involved with patient. Another possible unintended audiences could include other patients and even interns/students who are shadowing physical therapists. However if it is in the digital format then there are likely way less unintended viewers as the clinic staff are the only members with access.
      - Medical Necessity/Defensible Form
        
        This document is closely related to a patient intake form however this document goes into further detail to clearly iterate the patients story. These are intended to aid the physical therapist as the more information available to clinic the better the treatment can be overall. These documents often must be sent from the patients prior clinics such as hospital where surgery occurred. The intended audience is the new clinic staff including physical therapist however previous clinic staff often view them as it is often a long process to get documents sent over.
    - - A patient medication list is similar to the consent forms as they are given to patient so that the clinic has a written document with patients medication information. Most of the time these documents are viewed once by the physical therapist and then stored as the purpose is just to keep physical therapist aware of patients status. These documents much like any other patient document can have unintended viewers such as family and friends within clinic. The digital versions however are much less likely to be viewed by unintended people unless the patient has someone else complete the form which is a common event.
    - - Patient consent forms are simple documents consisting of descriptions of possible hazards of physical therapy. They are intended to be signed by the patient and then given back to the clinic so that they have written consent form the patient. Intended viewers are the patients and clinic staff however unintended viewers could vary based on whether it is a digital form or physical. Much like the other digital forms they are less likely to have unintended viewers whereas the consent forms can involve anyone within vicinity of the patent such as family and friends. Physical forms are often viewed by friends or family as new patients often do not travel alone.

These are often compared to progress reports as they can be valuable in seeing patients progression over the treatment. The physical therapist is the intended viewer as they are the one who use the information but like mentioned before patients sometimes like to look back on initial measurements to see progress. These documents are physical copies that the physical therapist can make important notes on.

This includes previous surgeries, medications, as well as basic information such as birthdate and name. The most common representation of these documents are physical copies filled and completed by patients at the clinic but more recently online versions have become popular and provide some advantages to both customer and clinic. The intended audience includes clinic staff and patient but could also include patients friends or family as it is common for patients have someone else fill them out.

The treatment plant guides the patients day to day routine but is intended to be used by physical therapist as they are the ones instructing the patients. Treatment plans are almost always physical copies opened by therapist every time patient visits. Unintended viewers may be those accompanying the patient such as family or friends. Treatment plans unlike any of the other documents are used every time the patient visits. Although the therapists create these plans, they are heavily influenced by past plans used to treat similar cases.
    - - At home exercises can be developed through the clinic or from other companies much like the diagrams and models. They are intended for the patient to use at home however they are often used within the clinic as well. Like many of the other documents, these can be presented in a digital format or printed on paper. An unintended audience may include the family member or friend of the patient who may inspect it. Another unintended audience may be a different clinic who has asked the patient to bring their previous records in.
- - - - Weekly schedules are used by the physical therapist and patient to determine when they will meet and for how long. They are typically written by the therapist based on the patient's availability however it is used by more than just the PT and patient such as family members or other workers within the clinic to help develop a time frame. This form of document is mostly used within the clinic but can be used outside the clinic by PT to look at future appointments as well as appointments in the past. Online schedules are often used now as they are easy to navigate and pull up quickly. Physical copies are sill used but for the most part they have been replaced do to digital revolution.
    - - Progress reports are written by the physical therapist and state quantitative measurements of the patients performance on a weekly basis. They are useful in how they show a progression of patients health and will ultimately determine when the patient will be done with therapy. Although it is intended for the patient it is often consumed by family members and insurance companies to provide information. It is also often used to keep track of past treatments and how the patient responded to those exercises. These documents seem to only come in physical forms as they are reviewed and signed by a therapist at the end of every week.
  - - - Models and diagrams are often developed from companies outside of the clinic and can be used to apply visuals to any number of situations such as routines or body parts. They are intended primarily to be used as a tool for the physical therapist to demonstrate ideas to the patient but they are also used by physical therapists themselves to improve their knowledge on a topic. Unlike traditional technical writing, models aren't described via text but instead are molded into shapes and often use multiple pieces to convey a message. Digital models and diagrams are a common occurrence nowadays. Online diagrams have the advantage of being navigated and altered to show better visuals whereas a physical copy provides the patients with hands on material for better learning.
    - - Workplace policies are a series of rules that must be followed within the clinic. They are produced by the company and are intended for all workers to follow however anyone within the clinic must also adhere to the written rules. Physical documents are often placed around the facilities so that everyone involved is aware of the information. Workplace policies can also be used in an online format where they can be sent digitally to all workers. Digital copies also have the advantage of being easily accessed from anywhere as well as being easily altered and shared.
- - - - The patient has been following a treatment plan by this point so it is important to keep track of how they are progressing. This is done by the Progress Report document. A progress report document also depends on previous treatments via the medical history docuements.
        
        Depending on the Patients insurance (provided by insurance form), the patients injuries (Medical necessity form), and the patients progress (progress reports) they have to determine how often and how long the patient must be treated. This is done by the Weekly schedule document.
        
        A patients or Physical Therapists use of models and diagrams is dependent on the exercises (Treatment Plan), as well as progress (Progress Reports) from the patient. Weekly schedules also determine how much time patients have to view models and diagrams.
