The patients evaluation is determined by there physical issues that are described in medical necessity form. The defensible document also provides evidence to allow treatment via treatment plans.
Patient consent is influenced by their prior treatments/experiences documented in a medical necessity form. Without new patients via brochures this document would not exist.
The medication list is similar to an insurance document as it determines how to approach creating treatment plans and determining what home exercises should be avoided.
Once a patient has been thoroughly evaluated via an evaluation form, a document to guide the patients treatment must be created resulting in Treatment Plans. The treatment plans involve exercises but a patient cannot take their treatment plan home so instead the clinic provides Home Exercises documents.
The patient has been following a treatment plan by this point so it is important to keep track of how they are progressing. This is done by the Progress Report document. A progress report document also depends on previous treatments via the medical history docuements.
Depending on the Patients insurance (provided by insurance form), the patients injuries (Medical necessity form), and the patients progress (progress reports) they have to determine how often and how long the patient must be treated. This is done by the Weekly schedule document.
A patients or Physical Therapists use of models and diagrams is dependent on the exercises (Treatment Plan), as well as progress (Progress Reports) from the patient. Weekly schedules also determine how much time patients have to view models and diagrams.

Insurance forms are derived from the patients medical history (Medical Necessity Form) and are also important in the patients decision of a clinic as not all clinics support all types of insurance. So patients should look at brochures to see types of accepted insurance.

A patient intake form is a staple for any medical field as they are interconnected with everything the patient has done and what the Physical therapist will do. An intake form and evaluation form are only necessary for new patients who are brought in via brochures.
Once a patient has completed the intake form it is necessary to get a understanding of the patients current physical status therefore an evaluation form is born. The evaluation is determined by the patients injuries and surgeries so can also be heavily influenced by a medical necessity form.
Brochures are responsible for attracting new clients. So if a new patient comes to the clinic they must then provide all the medical history forms as well as complete new forms such as patient intake form.
When a new client is interested in getting treatment, they should be informed of the workplace policies and the workplace policies cannot exist if there or no customers or staff.
