Pathologies
Hip OA
Progressive joint disease, Wareing of articular cartilage, changes to bone surfaces, sclerosis and osteophyte formation, ligament laxity, synovitis, joint space narrowing = pain, stiffness, feeling of instability.
Tendonopathy - Gluteal, adductor, hamstring, rectus femoris
From increased load or reduced tissue capacity, leading to changes within tendon eg. increase in prostoglycans and neurovascularisation and changes in collagen matrix.
Greater trochantic pain syndrome (gluteal tendonopathy)
Deep gluteal pain syndrome (prev Piriformis syndrome)
Nerve entrapment in deep gluteal space. sciatic nerve = sciatic distribution of pain, numbesss.
Laberal tear
Associated with FAI. pain, stiffness, clicking/clunking.
Femoroacetabular impingement (FAI)
Cam lesion = femoral head. Pincer lesion = acetabulum. degenerative changes to hip. often mistaken for groin strain.
Total hip replacement (THR)
Inflammatory arthropathies
RA or AS. Systemic multi-joint pathologies = hip pain and stiffness
Paediatric
Avulsion fractures. SUFE (age 10-13). Juvenile arthritis . leg calve perches (age 4-8)
