Teacher has a concern about a student: academic, emotional, behavioural
Teacher discusses student’s learning with Learning Community and parents and shares concerns
Teachers in the Learning Community identify concerns and develop strategies to trial.
Teacher documents strategies used.
Strategies lead to noticeable improvement so no further action required
Concern persists, teacher completes Student Support Referral Form and gives form to Student Support (SS)
Teacher informs parents of the referral made to Student Support
Student Support may observe student during a variety of activities and completes report of findings
A learner profile is established with Student Support in conjection with the Learning Community Teachers and Parents.
Student added to Support Register Stage 2
Parents are involved in the creation of a learner profile and, if appropriate, of strategies to try at home
Strategies implemented. Student Support and teachers responsible for observation and data collection and complete report of findings. Internal assessments may be carried out. (4weeks)
SST reconvened to discuss data collection.
Targets set (with success criteria and strategies) and regular, formal, review process implemented.
Parents are involved in the setting of child’s targets and, if appropriate, of strategies to try at home
Student shows satisfactory improvement, maintain support provided
Student shows significant improvement, student moved to Stage 1
Minimal progress noted. SST reconvened to consider carefully possible assessments to be carried out by outside agencies
Teachers and SST meet parents to recommend external assessment(s). If possible, parents to be given a list of possible providers
SST reconvened to complete Individual Education Plan (IEP) following recommendations provided by outside agency
Student added to Support Register Stage 3
