2⃣ Add major cause categories
These categories could be anything, depending on your problem, but examples include:
Measurements
Use the first-level branches of the mind map for broard categories of causes that contributed to the problem
1⃣ start by adding a short description of the problem being solved in the centre of the diagram
3⃣ Add detailed causes to each category
dive progressively down into the details by adding more branches

If a product was assembled incorrectly, and training has been identified as a cause category at the first level, the detailed causes might be:
Correct instructions from training were not observed
A large number of instructions are difficult to remember
the step in training was not given enough time
the training material wasn't clear enough
Training was given a long time ago with no refresher
The training did not include how to recognise mistakes and correct them
4⃣ Identify which causes should be addressed to solve the problem
Some may be easier than others to fix
And addressing just a few causes of the problem may be sufficient to solve it, without solving everything
