Referral form
Purpose: To collect all information initially required from the applicant
Collected: Applicant fills out the form on the website, copy emailed to a group inbox
Stored: In the group inbox, paper copy printed out
Processing: Data entered into Referrals spreadsheet, Alberon database, any documents attached are stored on the G:Drive
Users: Referrals coordinator, Executive Team member checking risk assessment, coaches, recovery staff
Reporting: QuaP Referrals Process report (demographics, service desired, method of referral, cluster number)
Automations: None at the moment. Forms need checking before they are entered on the database due to user errors
