Ethical Dilemma: Do or do not report incidents of severe radiation burns in patients. The physicians could either report the incidents which might indicate the machine is faulty and require repairs, or ignore the claims and observations and not report the incidents which could mean that patients would continue to be harmed.

Reporting the incidents is the right and ethical thing to do in these instances to prevent future harm to more patients. If the incidents had been reported initially, they could have investigated the machinery and possibly prevented those radiation overdoses.
