Personalisation means...
Recognising that the person has individual strengths, preferences, wishes and aspirations.
Means putting them at the centre of the process by identifying their needs and supporting them to make choices about the services they want so that they can live the way they want to.
Means individuals will have to decide which outcomes they wish to achieve, and how their needs and aspirations may be met.
Individuals are involved in all aspects of their care needs and must take part in all the decision making.
Is a proactive approach for the individual rather than the traditional approach which determined what services they would receive, and where and how.
Key Features
Personal budget - is an agreed amount of money that is used to carry out or deliver certain aspects of provision set out in an individuals support plan - 1) Direct Payment
2) Managed Account
Co-production - This is about collaboration or working together.
Partnership between citizens and public services (e.g. Neighbourhood watch and Healthy Schools).
Choice and control - a commitment to giving individuals more choice and control, supporting and enabling them to make their own decisions about where and how care is provided and the support they need to live a full and independent life. Individuals may decide, for example that they need to have better:
Self-assessment of needs - ed (where possible) by the service user.
The professional will talk to the individual about the support they need in different areas of their life.
Changing role of professional - The control has moved from the professional to the person. Professional must be supportive, sensitive and non-judgemental.

Personalised Care
The Department of Health (2019) defines personalised care as: … “every person who receives support, whether by statutory services or funded by themselves, will have choice and control over the shape of that support in all care settings”.
