Non-Cystic Inflammatory Lesinos
Acute Periradicular Periodontitis
Clinical Features
History of pain
caries
previous trauma
little radiographic evidence unless acute accerbation of chronic lesion
Histopathology
acute inflammatory changes - vascular dilation, presence of neutrophils and oedema
Treatment
XLA
RCT
Acute Apical Abscess
Clinical Features
Pain s
swelling sinus
pus
TTP
Histopathology
central collection of pus (neutrophils, bacteria, cellular debris)
adjacent bone of preserved neutrophils
surrounding membrane of sprouting capillaries and vascular dilation
fibroblasts
Treatment
Drainage
XLA
RCT
Chronic Periradicular Periodontitis
Clinical Features
Non-vital tooth, previous RCT
mnimlal symptoms
apical radiolucency
Histopathology
chronic inflammatory changes - lymphocytes, macrophages, plasma cells
granulation tissue
resorption of bone+/- tooth
Treatment
RCT or re RCT
XLA
Periradicualr sugery
Periapical Granuloma
mass of inflamed tissue at the apex of a non-vital tooth which can turn into abscess or undergo cystic chage
Symptoms
non-vital tooth
minimal symptoms
radiolucenecy at apex
Histopathology
inflamed chronic inflammatory granulation tissue
multinucleate foreign body giant cells
cholesterol and haemosiderin deposits from RBC and inflammatory cell breakdown
proliferation of cell rests of mallazez
resorption of bone +/- tooth
Treatment
XLA
RCT or ReRCT
Periradicular Surgery
Pericoronitis
Inflammation fo the soft tissue around a partially erupted tooth - exacerbated by opposing tooth
Clinical Features
pain
swelling
tenderness
bad taste
trismus
Histopathology
acute and chronic inflammatory changes - oedema, inflammatory cells, vascular dilation and fibrinotic CT
Treatment
irrigation
consider XLA opposing tooth
antibiotics if systemic symptoms
