Melanoma

Rarely, nodular and amelanotic melanomas may occur

Refer using suspected cancer pathway referral for melanoma if score 3 or more in 7-point checklist, or dermoscopy suggests melanoma or skin lesion suggests nodular melanoma.

Safety netting
Consider review of people with any symptom that is associated with an increased risk of cancer but who do not meet criteria for referral or other investigative action.

Squamous cell carcinoma
Usually seen by a raised lesion on the skin; a number of typical features of the lesion have been described.
Diagnosis: often possible to diagnose visually, but confirmation by excision biopsy.

Basal cell carcinoma
Typical features: ulcer with a raised rolled edge, prominent fine blood vessels around a lesion, or a nodule on the skin (particularly pearly or waxy)

Consider routine referral if lesion raises suspicion of basal cell carcinoma or suspected cancer pathway if delay may impact significantly due to lesion site or size.
7-Point Checklist
Minor features (1 point): largest diameter 7mm or more, inflammation, oozing, change in sensation.
Major feature (2 points): Change in size, irregular shape, irregular colour.
Refer using suspected cancer pathway for melanoma if they have a suspicious pigmented skin lesion with a weighted 7-point checklist score of 3 or more.
