Well, you should try to go to bed earlier or stop taking melatonin. You are becoming reliant on it. Try this and come back in a couple weeks to see if it helped.

Yes, it can have that effect if you use it every night. There are a few ways to stop your reliance on melatonin. We can do this by limiting caffeine intake to four to six hours before bed, making our bedrooms more comfortable through temperature-control, or training our brains to respond to light levels.
Oh interesting. I didn't think about my caffeine intake being a problem for my sleep. I just really like my coffee.
Well, good news then for you. You could always try decaffeinated coffee so that you can still get your sleep at night.

I mentioned changing the temperature of your bedroom. Room temperature between 60-67 degrees Fahrenheit can help you sleep. And I also mentioned training your brain to respond to different light levels. This can be done by having bright lights during the day and morning, then having dimmer lights during the night. Your body runs off something called the circadian rhythm which in your case could be keeping you awake if you are sleeping with lights on.
Oh, no no I don't want to do any of that. I think it might be my caffeine.
Well, I am glad I can be of help. How about you try different decaffeinated coffees and drinks to find out what you like then come back in, lets say in two weeks time to reassess how your sleep has improved.

You can start by reducing the amount of melatonin you have been taking. Maybe go from 10mg down to 5mg.

You are busy as a nurse but that doesn't mean you should dismiss a patients concerns and make assumptions about the problem.
This route isn't bad but you didn't ask questions to the patient to determine other possibilities. For example you missed out on the opportunity to educate the patient on their caffeine use which greatly increases their sleeping problems.
This route was good. Not perfect but good. You educated the patient, asked to reassess and learned about problems they were unaware were leading to a problem with sleep. Just because you know coffee has caffeine in it which can cause sleeping problems doesn't mean they are aware of it.
