Urinary Tract Infection: bacteria infect the lining of your urinary tract. This leads to inflammation and irritation, causing red blood cells to leak into your urine.

Kidney stones : The direct impact of stone on lining cells of the urinary tract causes destruction, thus allowing blood to leak into urine.
IgA nephropathy: deposits of IgA build up and damage the glomeruli. The damage causes kidneys to leak blood (hematuria) and protein (proteinuria) into urine.
Bladder cancer : Cancer cells that begin to grow in the inner lining of the bladder form tumors in the bladder lining, which can cause bleeding

Renal artery embolus , infarct, dissection or aneurysm
