Osgood-Schlatter disease
Definition: Osgood-Schlatter disease is an apophysitis of the tibial tuberosity that causes anterior knee pain during adolescence and is usually self-limiting. It is usually unilateral, but can affect both knees. The pathophysiology is not fully understood, but is thought be caused by traction of the patella tendon.

Biomechanical risk factors - muscle tightness, reduced flexibility.

Diagnosis

Gradual in onset, initially mild and intermittent but may progress to sever and continuous

Investigation

X-ray may be normal or may demonstrate anterior soft tissue swelling, thickening of the patellar tendon, fragmentation of the tibial tubercle or ossicle

Management
Pain relief - Paracetamol/NSAID if required, ice packs and protective knee pads.
Sporting activity can continue, but may need to reduce/modify for a limited period. If modified activities still provoke pain - limited period of avoidance may be required with gradual re-introduction.

Persistent symptoms: reassess possible cause of knee pain, refer for specialist assessment if diagnostic uncertainty. If symptoms persist into adulthood - advise use of protective knee pads may be helpful and refer for specialist review if symptoms significantly impacting function.
