Based on data from the Regional Health Research in 2017 in Indonesia, hernia is the second order disease after urinary tract stones as many as 2,245 hernia cases. The proportion of hernias in Indonesia is dominated by heavy workers at 70.9% (7,347), the highest is in Banten 76.2% (5,065) and the lowest is in Papua, namely 59.4% (2,563). In Indonesia, the infection rate for surgical wounds reaches 2.30% to 8.30%.

Pathophysiology

Indirect inguinal hernia occurs due to protrusion out of the cavity peritoneum through the internal inguinal ring which is lateral to the inferior epigastric vessels, then the hernia into the canal inguinal and if long enough, will protrude from the annulus external inguinal. If this bulge continues, it will reach the the scrotum in the same way as when the testes descend from the abdominal cavity to the scrotum.

Strangulated hernia

Multiple small bowel loops in the scrotum due to right indirect inguinal hernia. Compression of the penile urethra with Foley's catheter is noted.

There is an anterior abdominal wall defect through which small bowel loops and omentum are protruding out of the abdominal cavity in a C-shaped configuration. The bowel loops involved in the hernia show wall thickening.
