Upper Limb 1
Describe the attachments, nerve supply and actions of various pectoral and Scapulohumeral muscles
Pectoralis major
- Origin: (Proximal attachments) Medial half of clavicle and anterior surface of sternum, first seven costal cartilages,
- Insertion: (Distal attachments) Proximal part of humerus (lateral lip of intertubercular groove)
- Innervation: Medial and lateral pectoral nerves
- Action: Adduction, medial rotation, and flexion of the humerus at the shoulder joint
Pectoralis minor
- Proximal attachments (Origin): Anterior surfaces of the third, fourth and fifth ribs, and deep fascia overlying the related intercostal spaces
- Distal attachments (Insertion) Coracoid process
- Innervation: Medial pectoral nerve
- Action: Depresses tip of shoulder; protracts scapula
Serratus anterior
- Proximal attachment: External surfaces of lateral parts of 1st–8th ribs
- Distal attachments: Medial border of scapula - Innervation: Long thoracic nerve (C5, C6, C7)
- Action: Protracts scapula and holds it against thoracic wall; rotates scapula
Scapulohumeral muscles
Subscapularis
- Proximal attachment: Subscapularis fossa
- Distal attachments: lesser tubercle of humerus
- Innervation: Upper and lower subscapular nerve
- Action: Medially rotates arm, helps to hold head of humerus in glenoid cavity
-
Supraspinatus
- Proximal attachment: Supraspinous fossa
- Distal attachment: Greater tubercle
- Innervation: Supraspinous nerve
- Action: Abducts long arm along with deltoid
-
Infraspinatus
- Proximal attachment: Infraspinous fossa
- Distal attachment: Greater tubercle
- Innervation: Supraspinous nerve
- Action: Laterally rotates arm
-
Teres Minor
- Proximal attachment: Middle part of lateral border of scapula (axillary border)
- Distal attachment: Greater tubercle of humerus
- Innervation: Axillary nerve
- Action: Adducts and lateral rotation of arm
-
-
-
Deltoid
- Proximal attachment: Lateral end of clavicle, acromion, spine of scapula
- Distal attachment: Deltoid tuberosity
- Innervation: Axillary nerve
- Action:
- Clavicular (anterior) part: flexes and medially rotates arm
- Acromial (middle) part: abducts arm
- Spinal (posterior) part: extends and laterally rotates arm
-
-
Rhomboids
- Rhomboid major
- Proximal attachment: Lower end of ligamentum nuchae and spinous processes of C7 and T1 vertebrae
- Distal attachment: Posterior surface of medial border of scapula
- Innervation: Dorsal scapular nerve
- Action: Elevates and retracts scapular
- Rhomboid minor
- Proximal attachment: Spinous processes of T2-T5 vertebrae
- Distal attachment: Posterior surface of medial border of scapula from the root of the spine of the scapula to the inferior angle
- Innervation: Dorsal scapular nerve
- Action: Retract scapula and rotate its glenoid cavity inferiorly; fix scapula to thoracic wall
Trapezius
- Proximal attachments: Medial third of superior nuchal line; External occipital protuberance; nuchal ligament; Spinous processes of C7–T12 vertebrae
- Distal attachments: Lateral third of clavicle, acromion, spine of scapular
- Innervation:Spinal accessory nerve
- Action:
- Descending part elevates scapula
- Ascending part depresses scapula
- Middle part - retracts scapula
-
-
Levator Scapulae
- Proximal attachment: Transverse processes of C1 –C4 vertebrae
- Distal attachment: Superior angle of scapula; Posterior surface of medial border of scapula
- Innervation: Dorsal scapular nerve
- Action: Elevates the scapula

Lymphatic Drainage
- Lymphatics are lymph vessels which carry a clear interstitial fluid called lymph.
- The lymphatic system provides an alternative route for interstitial fluid to return to the venous system.
- Plays role in defense mechanism
-
-
Movement of shoulder
- Flexion
- extension, abduction, adduction, external/lateral rotation, internal/medial rotation and circumduction.
-
-
-
-
