Current feeding pattern. Bottle feed, formula, 55ml, three hourly.
Temperature- 37.4
Bowl motion.
Normal stools + Urine.

Dose- 5mg/kg = 15.3mg 36 hourly. Ensure adequate flossing and a one hour time difference between penicillin-containing compounds.
Due to the mother being taken to ICU for endometritis from an e-coli bacterial infection, the infant was placed on Gentamicin to aid in treating any infections that may have been passed on to the infant and prevent early septicaemia.
Used in combination with Amoxicillin for 48 hours as routine prophylactic antibiotic cover.
Used to treat systemic infections caused by susceptible gram negative organisms e.g. E coli, Klebsiella species, Pseudomonas species.
Monitoring
Take a trough level two hours before 3rd dose is due no trough levels have been taken prior to this. (Follow up the level prior to giving any subsequent doses).
In the first week of treatment aim for a trough level of <mg/L.
Repeat level before each 3rd dose (i.e. every 72 hours).
If the through level is >2mg/L- discuss with consultant or pharmacist as the dosing interval will need to be extended aor another antibiotic considered.

If renal function is impaired , levels will need to be taken earlier and or more frequently - Discuss with consultant or pharmacist.
