What to look out for: current issues w oral nutrition (aged care already would have been on modified diet), symptoms (food getting stuck, coughing on water, SOB when eating, globus sensations), when did it start, do they do anything to assist/minimise the issues, current diet (are they avoiding any foods), adequacy of oral nutrition (weight loss), any taste issues, pts', carer's concerns, medical diagnosis (urinary tract infection), stability of mental state, acquired/degenerative? Any issues w respiratory status (med Hx, observe resting respiration rate, O2 stats if monitored, habitual coughing), medication, history/presence of intubation, history/presence of non-oral feeding, cognition (ability to comply with, follow directions), self awareness of issues, comm status (able to resond/unds)? Oral condition, care, positioning in bed (medical brace/traction)
