Hospital Acquired Pneumonia (HAP)
is an acute lower respiratory tract infection acquired at least 48 hours after admission to the hospital
and is not incubating at the time of admission
incubation period is the period between exposure to an infection and the appearance of the first symptoms
How to check if patient has pitting edema
the doctor will push on the skin and measure the depth of the indention
they will then record how long it takes for the skin to rebound back to its original position (usually takes 1 - 2 minutes)
Hyperkalemia
Can be caused by drugs such ACEi, spironolactone
Can also be caused by food such as durian, banana, papaya

Urinary tract obstruction
AKI

How to counsel patient with HF
Restriction of fluid
Limit salt intake
Monitor BP at home or can go to the nearest pharmacy
Comply to his current medications
Avoid certain drugs such as NSAID which can cause fluid retention
Counselling points for antibiotic
Do not share antibiotics with other people
Inform the doctor if patient experiences any side effects
Take antibiotics on empty stomach
Finish entire course of the antibiotics
What can cause high level of creatinine
Drugs such as trimethoprim (but only temporarily)
High consumption of protein
AKI
Sign and symptoms of overwafarinisation
Bleeding from cavity
Severe headache
Blood in urine, blood in stool
Summary of the case
Bisoprolol
Start with the low dose - 1.25mg OD
ACEi
Need to titrate slowly, to avoid side effects since the patient is an elderly
ROF
Limit intake to 500ml per day/small bottle of mineral water
Anemia
Give ferrous fumarate to the patient to increase the hemoglobin level of the patient
