Assess cognition with change in orientation, people, places, times,range, attention, & thinking skills.

Orient patient,family, and caregivers on condition and process
Maintain regular fail schedule to prevent issues that result from tirst,hunger, sleep deprivation, and inadequate exercise.

Pay close attention to clients when the appear to be agitated, or engage in dangerous behaviors.

Help cargivers understand the patient might feel angry, relief, depression, fear, isolated, and a sense of loss.

Stage 1: Mild stage- trouble remembering the right words,name, performing task in social settings, forgetting newly introduced material, losing or misplacing material,and having trouble with planning or organizing.

Inform the patient to consider planning for the future i.e. legal, financial, designating a advocate,and safety plans
Stage 2: Moderate stage- inability to recall home address, telephone numbers,name of family & friends, confusion about time or place, difficulty choosing proper clothing for the season or occasion, and in need of assistance with eating and toileting.
Stage 3: Severe stage- loss of ability to communicate, normal movement,abnormal reflexes, muscle rigidity, and swallowing can become impaired.
