RAPE TRAUMA SYNDROME 8/6/2020 AR
PATHOPHYSIOLOGY
RTS-It refers to the post trauma victims of sexual assault present. It includes physical, emotional, cognitive, and interpersonal trauma.
MEDICATIONS
Tetanus toxoid: as prophylaxis if the patient doesn't have a booster immunization for the past 10 years. .
Medications options to prevent pregnancy if the victim wants to avoid it.
MEDICAL INTERVENTIONS
Provide immediate safety
Help reaching relatives
Give access to sexual assault advocate and social services
Obtain consent for physical, genital, and pelvic examination
Explain the procedure, and provide privacy
Collect evidence
Provide care for injuries, pain, molesting symptoms
Offer medication to prevent pregnancy
Provide confidentiality.
BEHAVIORAL INTERVENTIONS
Explain that nightmares, phobias, insomnia, anxiety, and depression are part of the phases of the trauma after a sexual assault.
Use therapeutic communication and let the victim express his/her feelings
Encourages the victim to express anger towards the assailant is necessary to promote effective coping
Support the survivor’s expression of sentiments
Avoid statements and interrogations that may be interpreted as attacking or blaming.
PSYCHOSOCIAL INTERVENTIONS
Emotional and behavioral responses
Defensive coping behaviors
Past coping mechanisms, including cultural, religious, and personal beliefs about assault.
Response of family members and significant others
Feelings of embarrassment or guilt about the assault.
IMPORTANT ASSESSMENTS
Sexually transmitted infections
Pregnancy
HIV Infection
Extent of injuries evaluation
Physical, genital, and/or pelvic examination
PATIENT EDUCATION
Arrange for support and follow up of: crisis counseling, group therapy, individual therapy, rape counselor, and group support.
Educate patient about the rape trauma syndrome
Educate patient about the medications needed for infections prevention
Teach patient about her options for pregnancy prevention
