Does the student have an IEP or 504 paperwork from another school?
No
Does the student show any warning signs, such as poor academic scores, behavioral difficulties, or communication problems? Do you, a parent, another teacher, or anyone else suspect the student of needing special education services?
Yes
Document and share your concerns with other faculty (and possibly parents). Consider evidence at a meeting with faculty and evaluate student's potential need for accommodations, modifications, or other services which may be provided by a 504 plan or an IEP.
If the team considers an IEP or 504 plan necessary, recommend student to an evaluation coordinator.
If the team determines that the student does not need an IEP or 504 plan, then attempt any minor accommodations that the team considers prudent and continue monitor the student for changes .
