Speech and language
Quantity
Talkative, spontaneous, expansive, paucity, poverty.
Rate
Fast, slow, normal, pressured.
Volume (Tone)
Loud, soft, monotone, weak, strong.
Fluency and Rhythm
Slurred, clear, with appropriately placed inflections, hesitant, with good articulation, aphasic.
Attitude
Attitude is assessing if the patient is aggravated, cooperative, focused or easily distracted.
LOC
Assessing for the level on consciousness.

Is knowing if the patient is aware of self, place, and time.
Behaviour
Mannerisms, gestures, psychomotor activity, expression, eye contact, ability to follow commands/requests,
compulsions.

What the patient is thinking about
Obsessions: Do you have intrusive thoughts or images that you can't get out of your head?
Phobias: Do you have an irrational or excessive fear of something?
Delusions: Do you think people are stealing from you? Are people talking behind your back? Do you think you have special powers? Do you feel guilty, as if you committed a crime? Do you feel like you are a bad person?
memory
A recent memory is being able to remember events from the recent past.
A remote memory is being able to remember events from the past of long ago.
Immediate memory is when a person can repeat back events in chronological order. Numbers, letters, or words given to them to repeat back can be asked to.
mood
A sustained state of inner feeling
To know what is their current mental status on how they are, if they feel energized, or angry lately.
insight and judgement
Patient's awareness and understanding of illness and need for treatment
thought process
Form of thinking, flow of thought
Anxiety, depression, schizophrenia, dementia, delirium, substance abuse
attention
Attend, concentration, distractibility.
“Spell the word ‘world.’ Now spell the word ‘world’ backwards.”
Calculations
“Add these numbers: (15 + 12 + 7)”
affect
An observed expression of inner feeling.

Appropriateness to situation, consistency with mood, congruency with thought content.
Fluctuations: Labile, even.
Range: Broad, restricted.
intensity: Blunted, flat, normal intensity.
Quality: Sad, angry, hostile, indifferent, tranquil, dysphoric, detached, elated, euphoric, anxious, animated,
irritable.
suicidality
Suicidality: Do you ever feel that life is not worth living? Have you ever thought about cutting yourself? Have you ever thought about killing yourself?
Assessing if the patient is a danger to self.
homicidality
“Do you think about hurting others or getting even with people who have wronged you?”
“Have you had desires to hurt others? If so, how?”
Assessing if the patient has thoughts about wanting to hurt others or wanting revenge.
