MENTAL STATUS EXAM 7/23/2020 AR
APPEARANCE
Is what we see in the client
For example: the posture,dress, grooming, and physical abnormalities.
ORIENTATION
Let us know if the patient is oriented to person, place, time and situation
We ask the client if he/she knows their name, where they are, and what time is it( season, time of the day, holliday.
BEHAVIOR
The eye contact
Psychomotor activity
Movements
ATTITUDE
It describes if the client is either cooperative, hostile, open, secretive, evasive, suspicious, apathetic, or distracted.
LOC
Level of alertness
For example: If the patient is awake, alert, lethargic, drowsy, and confused.
SPEECH & LANGUAGE
The rate of speech, and language
The rhythm
The volume
The content
The fluency
MOOD
The emotional state the client tells you
How the patient feel
For example: depressed, anxious, happy, angry, good.
AFFECT
The emotional state we observe in the client
The type
The range
The congruency
The stability
THOUGHT PROCESS
It describes the rate of thoughts
How thoughts flow and are connected
The normality
The abnormality
For example: coherent, organized, and not clear.
THOUGHT CONTENT
Refers to the themes that occupy the client's thoughts
For example: preoccupations, illusions, ideas, hallucinations, and despersonalization.
SUICIDALITY
It tell us if the client has intentions to harm him/herself
Or if the client has suicidal thoughts
We ask them questions about it like: have you ever thought of doing away with yourself?
HOMICIDALITY
It tell us if the client has intentions of harm others
We ask questions asking about the intentions
We can ask: Have you had desires to hurt others?
INSIGHT & JUDGEMENT
It tell us if the client knows about what is going on with him/her.
It also tell us if the client knows about his/her situation
We can ask: What seems to be the problem? or How would you describe your role in this situation
ATTENTION
It shows if the client is attending, concentrating, or distracted
We recite numbers to them and tell them to tell us the numbers back
We can also ask them to spell a word backwards
MEMORY
It tell us if client is able to keep short, immediate, and long term memory
We can ask them recently information
We can also ask them long term information such as their social security number.
We mention 3 words and ask them to tell us the same words 5 minutes after
INTELLECTUAL
This one refers to the knowledge of the patient about certain things we ask them about
We can assess their vocabulary and information
We can also ask them abstract questions like: to compare two objects and see if they answer on a abstract or concrete way.
