Integumentary system
∆ระบบปกคลุมร่างกายประกอบด้วย
1.ผิวหนัง Skin
ในผู้ใหญ่มีประมาณ 1.5-1.8 ตารางเมตา
มีเลือดมาเลี้ยง1/3
2.ส่วนที่เจริญมาจากผิวหนัง ได้แก่ เล็บ ขน ผม
3.โครงสร้างเสริม (Accessory Struture) ได้แก่ ต่อมเหงื่อ,ต่อมไขมัน,กล้ามเนื้อ,ต่อมน้ำนม,ต่อมขี้หูบริเวณหูชั้นกลาง
∆หน้าที่
1.ป้องกันอันตรายจากสิ่งแปลกปลอมและเชื้อโรคต่างๆ
2.ช่วยควบคุมอุณหภูมิของร่างกาย
3.ช่วยในการขับถ่ายของเสีย
4.เป็นอวัยวะรับความรู้สึกเกี่ยวกับสัมผัส อุณหภูมิ และความเจ็บปวด
5.ช่วยป้องกันอันตรายจากรังสี ultraviolet
6.เปลี่ยนPrecursor molecules เป็น Vitamin D
- ป้องกันการสูญเสียน้ำของร่างกาย
Skin layer ชั้นผิวหนัง
•เนื้อเยื่อที่อยู่ใต้ชั้นผิวหนัง(Hypodermis)
-Composed of areolar tissue
with abundant adipocytes
สีผิว (Skin color pigments)
•Melanin
-ถูกสร้างโดย melanocytes ซึ่งจะเปลี่ยน
tyrosine ไปเป็น melanin
-UV จากแสงแดดเป็นเวลานาน จะเพิ่มการสร้าง melanin ไปเป็น melanin
-ความแตกต่างของสีผิวแสดงถึงชนิดและปริมาณของ melanin ที่แตกต่างกัน melanocyte ของคนผิวดำหรือผิวน้ำตาลสร้าง melanin มากกว่าคนผิวขาว
•Carotene ในชั้นหนังแท้
-yellow-orange pigment สะสมในชั้น stratum corneum และไขมันในชั้นหนังแท้และชั้นใต้หนัง
•Hemoglobin ในชั้นหนังแท้
-อยู่ในเม็ดเลือดแดง มีคุณสมบัติจับออกซิเจน มี meranin น้อยทำให้มองทะลุหนังกำพร้า
Epidermis หนังกำพร้า
Stratum
•Basale
-Mitosis layer,continuously
replaces epidermal cells ,
turnover rate between 25 to 50be days
•Spinosum
-Consis of several layers of spiny-shaped cells
-cell are pushed upward and flatten out
•Granulosum
-Consis of 2-3 layers of flattaner cells
-Cells lose their nuclei
•Lucidum
-It is only 1or 2 deal cell layers with indistinct boundaries and translucent
•Corneum
-Consists of 25 or more layers of dead squamous cells still joined by desmosomes
•Functions
-Physical barrier to ligth , heat waver , chemical , microorganism
Cells
•Keratinocytes
-Most common cell of
the epidermis
-Cells readily divide
-Provides protection
and waferprofing
•Melanocytes
-Irregular shaped with
long processes
-Produces and transter the
protein to keratinocytes
•Langerhans cells
-Easily damaged by UV
-Arise from red bone marrow
and migrate to the epidermis
•Merkel cells
-They respond to touch sensation
Dermis หนังแท้
1.Papillary layer
-เป็นชั้นที่บาง อยู่ชั้นบนประกอบด้วย Loose CT
-พบผิวด้านยื่นขึ้นเหมือนนิ้วแทรกเข้ามาในชั้น epidermis คือ Dermal papillae
-Epidermis ที่ยื่นลงมาระหว่าง dermis papillae เรียกว่า epidermis ridge
-ในชั้นนี้จะมีปลายประสาทรับความรู้สึก
-หลอดเลือดฝอย ช่วยปรับอุณหภูมิของร่่งกาย
- Reticular layer
-คิดเห็น 80% ของความหนาของ dermis
-โครงสร้างเป็นDense irregular connective tissue
-มีblood vessels & nerves
-พบHair follicles & arrectorpili m
สิ่งที่เปลี่ยนแปลงมาจากผิวหนัง
∆Nails เล็บ
•เล็บเป็นแผ่น karatin แข็ง ที่เปลี่ยนแปลงมาจากชั้น

•semitransparent --- pink
-nail body
-nail root
-lunula
-stratum corneum
-ร่องระหว่าง nail fold กับ nail body
-ชั้น stratum corneum
ที่อยู่ใต้ free edge
∆Hair ขน
•พบได้ทั่วไปของผิวหนัง
ยกเว้น ฝ่ามือ ฝ่าเท้า
•ขยายสู่ชั้น dermis
•ขนแบ่งออกเป็นส่วน
-hair papilla
-hair root
-hair shaft
• โครงสร้างของขน ประกอบตัว 3 ชั้น
-cutcle
-cortex
-medulla
∆Hair Color สีผม
•Hair color is due primarily to the amount and type of melanim
-Dark hair contains true melanin
-Blond and red hair contain melanin with iron and sulfur added
-White hair air bubbles in the medullary shaft
•Hormones influence the growth and loss of hair

•Arrector pili กล้ามเนื้อ
•Sebaceous glands ต่อมไขมัน
•Apocrine Sweat Glands
•Eccrine (Merocrine) Sweat Glands
•Ceruminous glands
•Mammary glands
∆Burns แผลไหม้
-ไฟ,น้ำร้อนลวก,แสงแดด,ถูกรังสี,โดนกรด-ด่าง
-ดีกรีความลึกของบาดแผลไหม้
1.First degree burn
2.Second degree burn
3.Third degree burn
-การรักษา : ให้สารอาหารทางหลอดอาหาร และการเปลี่ยนของเหลว
∆Skin Cancer มะเร็งผิวหนัง
แบ่งออกเป็น 3 ชนิด คือ
1 Basal cell carcinoma
2 Sauamous cell carcinoma
3.Malignant melanoma
