Email the Learning Support Department for a 4-week initial intervention (NISC,2019) (See Link of the list of accommodations)
Initial Interventions successful?

Stop observation and monitoring and Maintain classroom instructions for the student
Subject teacher with the support of the Head of Department and Grade Level Leader submit Referral Form (NISC, 2019) (See link)
LS Teacher and Subject Teacher collaborate to implement and review existing plan as appropriate in a 4-week observation period
Initial Intervention Successful?

Stop observation and monitoring and Maintain classroom instructions for the student
No
Convene the Learning Support Team and the subject teachers for a meeting to develop ISP
Convene the Learning Support Team and the subject teacher to modify ISP
Implement new interventions following the tiered approach and collect data in a period of 6 weeks -- Here's an example of a school-wide collaboration for Special Education
Is ISP Interventions Successful?
Subject Teacher observes that the student struggles to meet the grade level standards (preferably in the first 6 weeks upon the student's entry) (NISC, 2019)

Stop observation and monitoring and Maintain classroom instructions for the student
Convene the Learning Support Team, the subject teachers, the counselor, and the Principal to intensify interventions and refer for evaluations
To refer for evaluation, convene the Learning Support Team, counselor and principal for a parent-teacher conference. Refer the student for testing, and once completed, LS Teacher and Subject Teachers reconvene to draft implementation of the IEP.
To intensify ISP, LS Teacher and Subject Teachers collaborate to formulate a new plan, implement it, and collect new data
Convene the Learning Support Team, the subject teachers, the counselor, and the Principal to determine whether re-evaluation is necessary

Implement the existing IEP
Convene the Learning Support Team, counselor and principal for a parent-teacher conference. Refer the student for re-testing, and once completed, LS Teacher and Subject Teachers reconvene to draft implementation of the new or revised IEP.
