Signs & Symptoms
recent infection
sputum production
chronic cough
bacterial colonization
SOB
haemoptysis
Non pleuritic chest pain
wheeze
inspiratory course crackles
large airway ronchi
Limited one area of lung/widespread
Causes
Other - foreign body aspiration, endobrochial tumours, rheumatoid arthritis etc
Congenital -marfans, Williams - Campbell syndrome
LRTI - pneumonia, pertussis, TB, Influenza
Diagnosis
Examination- course crackles lower lungs, wheeze, large air ronchi, finger clubbing
Investigations
Spirometry
Sputum Culture
CXray
Referral to respiratory physician
Examination
Risk Groups
COPD - frequent exacerbations
Children - asthma responding poorly to treatment
Infective exacerbation
acute deterioration
with or without increased wheeze, SOB, haemoptysis, or systemic upset
worsening cough with increased sputum
Abx- depending on local guidelines, or sputum cultures
Amoxicillin/Clarithyromycin/Erythromycin- Doxycyline
Complications
Resp/Heart Failure
Infective exacerbations
Pneumothorax
Low mood/isolation, tiredness, reduced QOL
Haemoptysis
tiredness
weight loss
Management
depression and anxiety
self mx plan
Increased sputum, discoloured, or change in viscosity
haemoptysis and/or fever
Increase cough and/or breathlessness
just in case ABx
Recognizing early signs of exacerbations
lifestyle advice- diet, exercise, smoking cessation
