Dysrhythmias: Disorganized Tachycardia - Coggle Diagram
Dysrhythmias:
Disorganized Tachycardia
VFIB
: a chaotic rapid rhythm in which the ventricles quiver and there is no cardiac output.
:warning:
WARNING
: :warning: VFIB =
fatal
if not resolved in 3-5 min
S/SX
NO pulse
NO BP
NO respirations
NO heart sounds
Interventions
Client is defibrillated immediately with 120-200 joules (biphasic) or 360 (monophasic)
check the entire length of the client 3 X to make sure no one is touching the client, when clear, proceed with defibrillation
CPR (until defibrillator is available) and ACLS
CPR is continued for 2 minutes and the caridac rhythm is reassessed to determine the need for further countershock
MEDS:
oxygen as prescribed
Antidyrsrhythmic therapy
Causes/Associated Disorders
Acute MI
Chronic diseases
cardiomyopathy
CAD
myocaridal ischemia
stimulation of the ventricles during procedures like...
cardiac catheterization procedures
cardiac pacing
reperfusion after thrombolytic therapy
accidental electrical shock
hyperkalemia
hypoxemia
drught doxicity
acidosis
AFIB:
multiple rapid impulses from many foci depolarize in atria; total disorganization of atrial electrical activity
350-600 times/ min
ATRIA QUIVER --> THROMBUS
Causes
:
Medical Tx:
Nursing Interventions
Instruct client in use of antidysrhythmic meds
Prepare client for cardioversion
Administer anticoagulants as prescribed
Administer oxygen
Chemical or electrical cardioversion
Antithrombotic therapy (thrombolytics); correction of rhythm
rate control (slow ventricular rate to 80-100bpm)
S/SX
Resp: trouble breathing, difficulty breathing when lying down, sensation of tightness in chest
Dizziness, fainting, confusion, fatigue
heart palpitations, irregular pulse (too rapid or too slow), racing, pounding or fluttering
Dysfunction of the sinus node, Mitral valve disorders, rheumatic heart disease, Pericarditis, and Hyperthyroidism
Hypoxia, HTN, CHF, CAD
Stable & unstable AFIB
