Factors influencing gastric emptying rate
Food
Meals that are nutrient-dense or that contain large fragments of food take longer to digest and will therefore slow the passage of drug into the small intestine.
Digested fat has a marked effect on slowing the rate of gastric emptying, particularly in the elderly.
Posture
When patients lie down on their left side, floating formulations (alginates) are presented to the pylorus and emptied first.
Lying down slows the rate of gastric emptying when compared with sitting up or standing.
Time of day
Gastic emptying follows a circadian rhythm, with faster emptying occurring in the morning compared with the afternoon.
Pregnancy
Gastric emptying is slower in women than in men.
Heartburn and constipation are associated with lower oesophageal sphincter pressure and decreased colonic motility.
Nausea and vomiting occur in 50-90% of pregnant women.
Alcohol
Strong alcoholic drinks (whisky) slow gastric emptying while beer/white wine significantly increase gastric emptying.
Smoking slows the gastric emptying rate of alcohol
pH
Digestion products of a high protein meal (peptides, amino acids) are potent simulators of acid secretion.
Disease states
Different disease states can alter gastric emptying rate.
