Any of these: vomiting, tachycardia, hypotension, fever (>102.5 or >1.8 from baseline), respiratory distress
STOP transfusion, check bag exp/type/for particulates, check patient vitals, check plasma for hemolysis

CXR, no infiltrates, hemolysis
acute hemolytic transfusion reaction:
supportive care
check cbc, tbili, BUN/creat, BP, PT/PTT

give more blood,
mntr citrate toxicity

Give O2, 1 dose lasix, CXR, tPOCUS
CXR infiltrates
