A 15 years old cat, listless, inappetent, thin, drinking more water, urinate large volume, vomiting frequently. The cat is moderately dehydrated, mucous membrane pale, kidney small and slightly irregular, hematocrit 22%( normal, 30-42%), serum creatine level is 8.7mg/dL ( normal, 0.5-1.2 mg/dL). The cat has chronic renal failure and common in old cat,serum creatinine level high as the glomerular loss function and reduced GFR. The urine is not cconcetrated in response to dehydration because tubular function compromised. The small kidney is due to gradual nephron loss. Anemia is common due to decreased erythropoietin production by the kidney.
