Middle ear disorders: Chronic otitis media
Definition
Long term, repeated, untreated acute otitis media becomes chronic if it has not cleared.
Pathophysiology
Eustachian tube swells and is blocked
There is negative pressure in the middle ear
Respiratory infection ascends to the middle ear
Fluid stagnates and there is no drainage
Ossicles become immovable
There is loss of hearing
Inflammatory advances
Fever increases and hearing decreases
Causes
Recurrent acute otitis media
Increased virulence of bacteria
Inadequate drainage in the eustachian tube
Upper respiratory infection
Nursing management
Dry ear by wicking
Administer antibiotic ear drops
Administer prescribed analegesics and oral inatibiotics
Health education
How to wick the ear
Discourage using cotton ear buds
Completion of prescribed antibiotics
Report recurrent infection or complications
Do not insert any objects in the ear
Keep the ear dry and clean
Protect the ear from getting wet
Report for follow up dates for check up
