The woman is concerned about then long-term effects of birth control pills and wants to stop taking them. They have two kids in high school and are certain they do not want more children. They seek your advice on what to do. What do you tell them?

I would recommend sterilization because it is the most reliable with the least amount of side effects. I would also advise them that male sterilization is the fastest, safest, and cheapest method.
Contraceptive methods:
Condoms: Create a physical barrier to prevent sperm from entering the vagina. 15% failure rate. Side effects are typically limited to skin irritations and reaction due to an allergy.
Typical birth control pill: Hormones in pill typically prevent ovulation which means that fertilization can not occur. Side effects such as weight gain, water retention, and blood pressure rise are common.

IUD: use copper or hormones to prevent pregnancy. small failure rate of 1%. Side effects include: Menstrual bleeding, irregular periods and PMS, uterine perorations, PID.
Sterilization of either male or female. It should be noted that while females more commonly get sterilized, vasectomies are faster, cheaper, and safer. They both have a less than 1% failure rate.
She is on the pill and in her 40s and knowing that taking estrogen can contribute to breast cancer, it might be best to choose an alternative.
