Chronic Thyroiditis (Hashimoto's disease)
Definition
the immune system develops antibodies that attack the thyroid gland
which makes the inflammation persists.
AKA Hashimotos’s disease or chronic lymphocytic thyroiditis
Causes
Autoimmune disorder: own self antibodies damage thyroid gland
Factors: heredity, sex and age may determine probability for development
Clinical Manifestations
Contradicts acute thyroiditis most symptoms
Fatigue & sluggishness
Sensitivity to cold
Constipation
Pale & dry skin
Hoarse voice
Pain and joints stiffness
menorrrhagia
depression
(Complications):
May present rapid growing nodules
May lead to thyroid lymphoma
May lead to hypothyroidism or goiter
Pharmacologic Therapy
Treatment is the same as that of hypothyroidism and goiter
Hashimoto’s disease causes thyroid hormones deficiency
Thyroid hormones replacements are required
Synthetic thyroid hormone: levothyroxine
Levoxyl & sythroid
Iron supplements: multivitamins with iron
Cholestyramine to lower cholesterol levels due to impaired metabolism
Calcium supplements
Management
Surgery is performed if pressure symptoms resists
Nursing Management
Nursing diagnosis
Risk for infection
Nutritional imbalance
Stress & anxiety
Hoarseness of voice
Nursing intervention
Monitor vital signs every 4 hours
Check temperature and report if above 38.3 °C
Assess areas that may introduce infection into the patient’s body
Monitor IV lines every 2 hours
Maintain sterility for every invasive procedure
Provide health diet as ordered by dietician
Provide emotional support to the patient
Observe for any difficulty in breathing
Keep tracheostomy at the bedside for in case indicated
Evaluation
Patient does not develop nosocomial infections
Vital signs are stable and within the normal ranges
The patient’s nutritional status is equal to the body requirements
Patient demonstrate less or no stress and anxiety at all
Hoarness of the voice subsides
