Nephrotic Syndrome
-
-
-
-
Nursing Goal
-
-
◦ Altered nutrition: less than body requirements related to anorexia and altered metabolic state secondary to renal failure
-
-
-
Therapeutic management
-
-
◦ If not respond to steroid therapy, immunosuppressive
medications will be considered e.g. cyclophosphamide, ◦ Reducing fluid retention in the tissues
-
-
-
◦ Dietary restriction include a low-salt diet and in more severe cases, fluid restriction
Nursing Management
-
-
◦ BW daily, assessment for edema
-
-
◦ If hypoalbuminemia, administer IV albumin
-
-
-
-
-
Preventing Infection
-
-
◦ Administer prophylactic antibiotics, if prescribed (e.g. Penicillin V)
◦ Live vaccines should be delayed until at least 2 weeks after corticosteroid or other immunosuppressive medication therapy ceases
◦ If unimmunized, notified immediately if exposed to chickenpoxmay administer varicella zoster immunoglobulin.

type 2 DM
(NIDDM)
-
is characterized by ain beta cell activity, uptake of insulin by peripheral muscle cells or an increase in glucose production by the liver. Overtime, the beta cells of the pancreas fail and exogenous insulin injections may be required.
-
polyphagia, polydipsia, polyuria are minimal as the pancreas still maintains some capability to produce and secrete insulin.
-
type 1. DM
(IDDM)
-
-
characterized by polydipsia, polyphagia, polyuria, increased frequency of infections, loss of weight and
strength, irritability and often ketoacidosis because there is no /low insulin secretion from the pancreas.
-
-
Insulin dosage adjustment is easily affected by inconsistent patterns of physical activity and dietary irregularities.

Urinary Tract Infection

-
-
Prevention
-
-
◦ Check for vaginitis or pinworms, especially if child scratches between legs
-
-
-
-
◦ Drink cranberry juice to acidify the urine. Avoid colas and caffeine, which irritate the bladder
