Teacher notices student is below expectations academically, behaving out of character, or acting out.
Record observations of students progress and keep track of this.
If the trend continues, contact administrators and parents to discuss
Form a personalized plan for the student to help promote success
Small group interventions
Try presenting information in ways that stimulate different senses.
Let the student try to complete assignments in ways that interest them such as drawing, dancing, singing, building, etc.
Communicate activities that can be done at home to help the student
Is the student improving?
Yes
Continue with the methods determined. If they stop being effective revisit new ideas or continue through the next stages of the process.

Communicate to administrators desire for formal assessment. Contact parents for permission.
Parents agree to formal observation from outside consultant
Based off of the consultation form a plan of action to assist the child further in class, bring in outside interventionists, and provide information/resources to parents.
