a disease that results in decreased bone mass and leads to weakened bones that are prone to fracture.
Diabetes can harm the kidneys by causing damage to blood vessels inside your kidneys. Over time, high sugar levels in the blood can cause these vessels to become narrow and clogged.
High blood pressure can cause arteries around the kidneys to narrow , weaken or harden. Once damaged they may not be able to deliver enough blood to the kidney tissue.
Osteoporosis often coexists with renal osteodystrophy, contributing to the fragility of bones.
High blood pressure can increase the volume being filtered meaning that filtration is sped up and not everything ends up being filtered.

With osteoporosis, if you do not have enough bone mass to produce healthy erythrocytes then your kidneys have more unhealthy erythrocytes to filter through and thus the bones don't get enough healthy filtered blood.
She is most likely seeing blood in her urine because diabetes can cause damage to the kidneys and cause albumin to pass through filters and end up in her urine. High blood pressure adds to the damage of vessels in her kidneys, making them deteriorate faster.
