Some mentors may feel that a fail mark may reflect their lack of ability to create an appropriate learning environment, facilitate teaching and provide appropriate feedback, thus highlighting their inadequacy as a mentor. Issues of insufficient time (Fraser et al 1998, Dolan 2003,

Kathy Duffy told the RCN practice education forum conference that mentors find it traumatic to fail a student. ‘One of the mentors in my study said it shook her confidence,’ Ms Duffy explained. ‘Another said: “I felt it must be me”.’
Mentors often blame themselves when they have to fail a clinical placement student
mentors are often challenged by personal and organisational conflicts, at the same time with further responsibilities such as managing a clinical workload in addition to being accountable and responsible for student learning and assessment.
