Treatment Refractory: 3 Adequate antidepressant trials and 1 mo. with an adjunct (antipsychotic, clonazepam, lithium, or Buspar) and at least 20 sessions of ERP
Legend:
ERP = Exposure & Response Prevention
CT = Cognitive Therapy
ACT = Acceptance & Commitment Therapy
DBS = Deep Brain Stimulation
ECT = Electroconvulsive Therapy
TMS = Transcranial Magnetic Stimulation
ECG changes with Anafranil or Antipsychotic
Obtain baseline ECG and BP
Repeat ECG as needed & monitor BP/HR at each appt.
D/c medication and switch to another agent
Serotonin syndrome at any time point?
D/c & switch to another SSRI
D/c & switch to Anafranil
D/c & switch to Effexor or Remeron
