The stomach
Structure

Divisions
-
-
-
Pylorus
Connects stomach to duodenum
3 areas: antrum, canal, sphincter
Sphincter demarks transpyloric plane (L1)
Curvatures
Greater
Long, convex, lateral border
Starts at cardiac notch, arches back, passes inferior to left
Curves to right and continues medially to pyloric antrum
Lesser
Short, concave, medial border
Inferior part is the angular notch, demarks junction of
body and pylorus
Attachment to hepatogastric ligament
-
Omenta and sacs
Two layers of peritoneum
folded over (4 membrane layers)
Attach to stomach
Omenta
Lesser omentum
Structure:arises at lesser curvature of stomach and folds
back to attach to the liver
Function: attaches stomach and duodenum to liver
Greater omentum
Structure: hangs from greater curvature of stomach and folds back upon itself to attach to transverse colon
Function: GI immunity, contains LNs, adheres to inflamed areas minimising spread of intraperitoneal infection

Vasculature
Arterial
Coeliac trunk and branches
Anastomoses along lesser curvature (right and left gastric)
and along greater curvature (right and left gastro-omental)
- Right gastric (from common hepatic)
- Left gastric (direct from coeliac)
- Right gastro-omental (gastroduodenal, from common hep)
- Left gastro-duodenal (from splenic)
Venous
Right and left gastric veins (to hepatic portal)
Right and left gastro-omental (to superior mesenteric)
Short gastric (to superior mesenteric)
Innervation
-
Sympathetic
Coeliac plexus (from greater splanchnic, from T6-T9)
Lymphatics
Gastric vessels drain to gastric and gastro-omental LNs, then to coeliac nodes on anterior abdo wall
