Step2: Homeroom collects enough information about the concern, and do reaserch about the concern.
Step3:Contact with problem solving team. Show the evidence and
reaserch result to the team.
Step4:Problem solving team will give strategies to the teacher according to what the concern is
Step 5:The teacher uses the strategies for 12 weeks and do documentation
Step 1: The parents and other teacher tell the homeroom teacher their concerns or the homeroom teacher has concerns from sings such as:
(1) Not meeting the benchmarks as other students
(2) Language problems
(3) Can’t write as well as other kids
(4) Extreme behavior
(5) Little progress
Step 6:The teacher meets again with the problem solving team.
Step 7.2 If the strategies don't work, the problem solving team will give more strategies for the teacher to use for another 12 weeks.
Step 8.2 If the strategies still don't have any effect on the student, the problem goes to the referral.
Step 10 Referral sends meeting invitaion to parents 7-10 days before the meeting.
Step 11.2 I the parent doesn't want to attend the meeting, send te report or do the testing, the problem will go back to the problem solving team.The problem solving team will give strategies for teacher to help the kid in the classroom.
Step 11.1 If the parent wants to attend the meeting and agree to send in the report from a psychatrist or do a testing at school, they will get the report from the parent or d the testing first.
Step15 Meet with the parent when they want to meet again.
Step 14 Use the pathway that the parent chose to help the kid.
Step 13 Meet with parents to give pathways for the parents to chosse from.
Step 16 After one, the referral must meet with the parent again to sep up next goal.
Step 17 Every three years, the referral must meet with the parent to do reevaluation and do another IEP.
Step 12 Make IEP plan for the kid.
Step 9 Homeroom teacher collects information such as:attendance, report cards, I station, NCENS (Math ability), universal screener to the referral.
Step 8.1 After 12 week,if the strategies work well, keep using them. The problem solving team may add strategies or remove stategies until the concern is no longer there.
