Definition
Compression of the heart from an accumulation of fluid
Pathophysiology
Reducing cardiac output leading to shock
It is associated with metastasis from cancers surrounding the heart
The compression restricts cardiac expansion and filling
Radiation reduced pericarditis may also lead to pericardial effusion
Signs and symptoms
Tachycardia
Narrow pulse pressure
Cold extremeties
Distended neck veeins
Decreased level of consciousness
Muffled heart sounds
Hypotension
Globular shape of the heart on chest x-ray
Nursing interventions
Identify and treat the underlying causes
Maintain ventilation and oxygenation as indicated
Give prescribed fluid therapy
Prepare patient for pericardiocentesis
