chlorhexidine baths / around port

Proper daily care per protocol, flushing line with blood return per protocol and checking dressing condition / condition of site

Basic hand hygiene before accessing port

Changing of alcohol impregnated caps every time port is accessed

Staff being able to identify infection while assessing site

Patient education

Patient set up with at home services to monitor central line condition and dressing changes

Patient and or family member understand hand hygiene before accessing central line

Patients risk for infection with having central line

Patient understands what can be into the central line at home such as medications and TPN

Patient understands how to care for central line

Patient recognize signs and symptoms of infection

Swelling or warmth

Redness around the site or red streaks around the site
