Special education referral process

Step 1: Teacher talks to students about their concern and to see if student can offer any information regarding the concern.
Step 3: Teacher identifies and logs an academic concern on school platform- Unified Classroom.
Step 4: Teacher tries their own differentiation strategies in the classroom. Refer to:

Step 5 A: No more concern. Teacher continues to differentiate instruction in classroom.
Step 5 B: Teacher Logs another concern and collects other data such as MAP scores, grades from other classes and comments from other teachers.
Step 6: Teacher seeks support from student support teacher for the grad level for further strategies and accommodations to use.
Step 7 A: No more concern. The teacher continues to differentiate and accommodate instruction in the classroom for student as per student support advice. The teacher continues to monitor student and provides feedback to the student support teacher and the academic councilor.
Step 7 B: Strategies are unsuccessful. Teacher completes the student of concern form with details of interventions tried and steps taken. The form is sent to the student support team and parents are notified.
Step 8: Student Support Team reviews the concern form and decides as a team the next steps.
Option 2: Gain Parental consent & Refer student for an ed-psych evaluation and assign student to receive Tier 2 + services.
Step 9: Create IEP based on student evaluation report and adjust Tier accordingly. Meet with parents to discuss IEP and next steps.

Option 1: Put an action plan and support system in place with more interventions and assign student to receive Tier 2 services.
Step 9: Communicate in person the action and support plan to teachers. Monitor student progress.
