Surfactant
-
An important part of lung development in babies is the production of surfactant. This is a substance made by the cells in the small airways and consists of phospholipids and protein. By about 35 weeks gestation,
most babies have developed adequate amounts of surfactant. Surfactant is normally released into the lung tissues where it helps lower surface tension in the airways. This helps keep the lung alveoli (air sacs) open. Premature babies may not have enough surfactant in their lungs and may have difficulty breathing. :
Symtomps
Intercostal Retraction RR >60,
grunting or moaning ,nasal flaling ,Apnea
Prophylasis
Prenatal use corticosteroid

Nursing Diagnosis
-ประสิทธิภาพการแลกเปลี่ยนแก๊สลดลงจากขาดสารsurfactant -เสี่ยงต่อภาวะอุณหภูมิกายสูง/ต่ำกว่าปกติจากศูนย์ความร้อนส่วน Hypothalamusยังพัฒนาไม่เต็มที่ -ได้รับสารอาหารไม่เพียงพอกับความต้องการของร่างกายจากคลอดก่อนกำหนด - เสี่ยงต่อการติดเชื้อจากการไดรับอิมมูโนกลอบบูลินจีเอ และเอ็ม จากมารดาไม่เต็มที่ และได้รับinvasive procedure( ETT, UAC/UVC)
- ส่งเสริมBonding -เฝ้าระวังภาวะแทรกซ้อนจากคลอดก่อนกำหนด(NEC,ROP,BPD)
