Pathophysiology
Lymphocyte and plasma infiltration occurs with local erosion of bone at the attachments of the intervertebral and other ligaments which heals with new bone (SYDESMOPHYTE) formation

Clinical Presentation

Pain radiates from the sacroiliac joints to hips, usually improves towards the end of the day
Loss of lumbar lordosis (normal inward curve of spine) and increased kyphosis (curvature of spine - causes top of back to appear more rounded than normal)

Typical patient is a man, <30 years, gradual onset of low back pain, worse at night & better with exercise, spinal morning stiffness
Non-articular features - anterior uveitis, associated with osteoporosis
