Gastritis (Causes (Specific causes e.g. Crohn's, Mucosal ischaemia -…
Gastritis
Causes
-
Mucosal ischaemia - reduced blood supply to mucosal cells can mean less mucin produced so acid can induce gastritis
Duodenogastric reflux - whereby bile salts enter stomach and damage mucin protection resulting in gastritis
Increased acid - can overwhelm mucin resulting in gastritis, stress can increased acid production

-
Treatment
H. pylori eradication
Confirmed by breath/antigen, use triple therapy (PPI + two antibodies)

Pathophysiology
Autoimmune - atrophic gastritis & loss of parietal cells, leads to pernicious anaemia
Drugs - inhibits prostaglandins & COX, less mucus production
-
Key Facts
-
Gastropathy indicates epithelial cell damage and regeneration WITHOUT inflammation - commonest cause in mucosal damage associated with aspirin/NSAIDs
Prevention
Give PPIs alongside NSAIDs - this also prevents bleeding from acute stress ulcers and gastritis which is often seen with ill patients - especially burns patients
