Phlebotomy Ch.10: Lecture 2
Bleeding Time Test
measures the length of time required for bleeding to stop after incision is made
a screening test to assess the integrity of the vascular system and platelet function
normal is 2-10 minutes
Abnormal Results
if it takes 15-20 minutes can mean pt has a condition that interacts with normal platelet plug formation or test was done
repeat test if only one cut was made
if two cuts were made and are within minutes of each other; don't repeat test
Procedure
Assemble equipment
alcohol pads
bp cuff
BT device
stopwatch
filter paper
bandage
Prep pt
tell pt scarring may occur
ask of aspirin or products containing salicylates taken in the last 7-10 days
or ibuprofen within 24 hrs
both could inhibit platelet function
position arm, select, and clean site
select a site 5 cm below the antecubital crease
make sure free of veins, scars, hair, and bruises
clean with alcohol and allow to dry
apply Bp cuff
inflate cuff to 40 mmHg
maintain that pressure throughout whole procedure
wait 30-60 seconds after inflation to make puncture
Position device parallel to crease of arm and 5 cm or below
Make incision and start timer
use firm pressure on skin
activate device and start device at same time
remove device after blade retracts
wick blood every 30 sec
touch only edge of the drop until it disappears and you can see the site
complete test
bleeding has stopped when blood no longer absorbed by filter paper
record time and remove cuff
Attend to pt
clean arm and don't use alcohol
apply butterfly bandage and tell pt to keep on for 24 hours
pull incision edges together when applying bandage
Ancillary Blood Glucose Test
done at pt bedside
for monitoring pts with diabetes mellitus
follow procedure from manufacturer's instrument used
always make sure to calibrate and run controls prior to testing the pt
know how to work critical value
critical for accurate results
