Diabetic Neuropathy

-
Clinical features
Insensitivity
-
Sensory loss can result in foot insensitivity which in turn can result in foot ulceration, infection, amputations and falls
Mononeuritis multiplex
-
Characteristic feature of diabetic CN3 lesion is that pupillary reflexes are retained owing to sparing of the pupillopmotor fibres

Pathophysiology
Isolated mono neuropathies are thought to result from occlusion of the vasa nervorum (small arteries that provide blood supply to the peripheral nerves)
More diffuse neuropathies may arise from accumulation of fructose and sorbitol (metabolised from glucose in peripheral nerves) which disrupts the structure and function of the nerve
