Ascending Cholangitis
Epidemiology
Ascending/acute cholangitis is an infection of the biliary tree and most often occurs secondary to common bile duct obstruction by gallstones (chiledocholithiasis)
Causes

In the Far East and Mediterranean biliary parasites can cause blockage and ascending/acute cholangitis

Clinical presentation
Fever (with rigors), jaundice (preceded by abdominal pain), RUQ pain - may not always be present
Jaundice is cholestatic thus there is dark urine, pale stools and skin may itch

Treatment
Urgent biliary drainage using Endoscopic Retrograde Cholangio-Pancreatography (ERCP) with sphincterotomy (cutting of biliary sphincter):
