        Request a Student of Concern meeting with the team lead for grade level and counselor. Make sure evidence and data is ready for meeting
        
        SOC team re-meets to look at evidence of interventions, solutions and date and determine if the student tis making progress. The evidence can include: student work, behavior, plan results, assessments and qualitative information. If progress is made, continue to monitor with SOC. If only some progress is being made, a new plan is created and SOC team re-meets in 6 to 8 weeks.
        
        At SOC problem solving with team leader/ counselor, advisor, teacher. Discuss already implemented strategies and new ones
        
        Using SOC document in powerschool, the SOC team creates a plan of intervention/ strategies for the student and identifies who will do what and a follow up date in 6-8 weeks. An SOC/SRT Alert with plan of interventions/ strategies is created

        The SOC Team brings the student of concern to the Student Success Team (what is used to be referred to as SRT) by compeleting the SST Referral form and informing Principal/ Assoc Principal
        
        The SST determines who will share the recommendations with parents, student, and other faculty as soon as possible after the meeting. Principal is usually part of the meeting
        
        The Principal or designee schedules the SST, comprised of classroom teachers, counselor, lSS and possibly parent or student
        
        The SST creates a plan and recommendations. This plan may require additional SST meetings Components of the plan may include Tier 2 and Tier 3 Interventions. Tier 3 Interventions are coordinated with the Principal and Director of Student Services.
        
        The SST reviews tee data and information from the SOC team that is shared in the SST Referral form and problem solves around solutions that might be more interventions and strategies, resources, counseling support, assessment, LSS or outside referrals
