DM II
Causes increased blood glucose levels and can result in glucose being excreted from the body in urine as a form of waste.

DM II can be overworking the kidneys by adding additional component (glucose) to be secreted in the urine.
Hypertension leads to increased pressure in kidneys and elevated force of flow through loops of henle and prevent proper reabsorption of calcium which could worsen her low bone density even further than osteoporosis alone.
All the ailments she has exacerbate each other. The DM II worsens the kidney disease, the kidney disease worsens the osteoporosis, and the kidney disease worsens DM II by not being able to effectively remove excess glucose from the body.
The likely cause of the blood in the urine is improper filtration, and possibly a UTI, which will be more difficult for this patient to overcome and adding antibiotics will cause additional strain on the kidneys.
